In order to help people better document their medications in case of a medical emergency, Beebe Medical Center in 1999 began distributing File of Life Packets. The File of Life card enables Fire/Police/Emergency Medical Services (EMS) personnel to obtain quick and accurate medical information. The card also eliminates the risk of a patient forgetting to mention something important. The information card is kept in a red plastic pocket labeled FILE OF LIFE held with a magnet for hanging outside of the refrigerator. A window sticker is visibly displayed outside the home so paramedics know to look for these packets. The card lists the patient's name, emergency medical contact, insurance policy, social security number, health problems, medications, dosages, allergies, recent surgery, religion and a health care proxy. The entire packet is Free and available at Milton Police Department, your local fire department, or EMS agency.

Please make sure that the original Emergency Medical Information form is updated. And, please complete the new universal Medication form and include it in your File of Life packet.
